What is Presbyopia?
Presbyopia, or age-related farsightedness, occurs due to the weakening of the natural accommodation of the eye lens, resulting in high prescription. The first symptoms usually appear around the age of 40, with the need for reading glasses, and it progresses into cataracts or cloudy vision in your 70s.
Over the years, various methods have been used to treat this condition, but today, the most effective treatments are laser procedures like classic monovision and Presbyond, as well as refractive surgery involving the replacement of the natural eye lens with an artificial one. The latest lenses with extended focus allow clear vision at all distances.
What is Presbyond Blended Vision?
Presbyond Laser Blended Vision is an advanced monovision laser treatment used to address presbyopia.
With the help of the Presbyond laser, sharp vision is achieved at all three distances (near, intermediate, and far) thanks to the “Blend” zone. This zone creates overlap, allowing the eyes to work together, providing clearer and more natural vision.
According to literature, approximately 95% of patients are suitable candidates for the Presbyond treatment.
Is Presbyond Blended Vision better than classic Monovision?
Classic monovision provides good distance vision in one (dominant) eye, while the other (non-dominant) eye is corrected for near vision. However, intermediate distances remain somewhat blurry, and depth perception is compromised.
According to literature, about 50% of people adapt well to the monovision method.
Presbyond offers many advantages compared to classic monovision:
- Better vision at all distances: Presbyond provides clear vision at far, intermediate, and near distances, while monovision only corrects one eye for distance and the other for near vision.
- Long-lasting near vision: Presbyond ensures good near vision throughout life. In contrast, classic monovision cannot guarantee lasting near vision sharpness, as aging of the eye and the growth of the diopter over time reduce the effectiveness of the correction.
- Safer driving: With the Presbyond method, there are no issues with driving, especially at night. Monovision, however, may cause difficulties due to poor depth perception.
- No need for artificial lenses: This method does not require the implantation of artificial lenses while the natural lens of the eye is still clear (RLE).
Candidates
This is when you begin to feel the need to push reading material away. The correction is typically achieved by wearing glasses with “plus” lenses, starting from +0.75 diopters, though the prescription can increase to higher values, such as +3.0.
Good candidates for the Presbyond treatment are individuals over the age of 43, who experience age-related farsightedness, up until their 60s and 70s, when vision sharpness begins to decline due to cataract development.
In such cases, it is possible to remove the cloudy lens and replace it with a lens that has extended focus or a monofocal lens if near vision remains good.
According to literature, after undergoing the Presbyond Blended Laser treatment, more than 98% of patients can comfortably read newspapers, and about 90% can read small print, such as the text on medication bottles and instructions.


Procedure

Laser vision correction at our clinic is performed on an outpatient basis, following the principles of same-day surgery. This means that after the procedure, there is no need for hospitalization, and the patient is immediately ready to return home.
Before the surgery, an eye examination is performed without the need for preoperative testing, as the procedure is carried out using eye drop anesthesia. This ensures local anesthesia without the need for general anesthesia. During the surgery, the patient feels no pain, only a mild touch on the eye.
The procedure itself takes between 10 and 15 minutes and is completely painless, with no need for stitches or sutures.
Eye Examination for Presbyond
Whether you are a candidate for laser vision correction is determined by an ophthalmologist through a detailed preoperative examination. The process starts with assessing visual acuity using a device that measures refractive errors. The curvature and thickness of the cornea are measured, followed by subjective testing to determine how well the patient can see.
During the examination, the dominant and non-dominant eye are identified.
Next, intraocular pressure is measured, the stability of the tear film is assessed, and pupil dilation is performed to examine the eye lens and retina. Finally, refractive error is measured again using wide pupil examination, also known as skiascopy.
This detailed examination lasts approximately one hour.
Preparation
There are no strict guidelines for preparation before the surgery, but we recommend the following helpful tips:
The night before the surgery, wash your hair, as during recovery you should avoid bending and contact of the eyes with water.
Wear a buttoned shirt, sweater, or blouse to avoid having to remove clothing over your head, thus protecting your freshly operated eye.
Prepare meals in advance, as it’s not advisable to expose your eye to heat or steam in the kitchen shortly after the procedure. You can eat normally before the surgery and take your regular medications.
Bring someone with you to take you home or arrange for a taxi ride, as public transportation is not suitable due to the risk of injury or infection to your freshly operated eye.
Presbyond Procedure Overview

Anesthetic eye drops are then applied to ensure the procedure is completely painless.
Once the drops take effect, the surgery can start. Each eye is treated separately, and the entire procedure takes around ten minutes per eye. It is quick, precise, and safe.
The operation is done in two steps. First, a small flap is created on the cornea using a FEMTO laser. This flap is gently lifted to allow the EXCIMER laser to reshape the cornea and correct the refractive error.
Thanks to advanced technology, there is no risk of eye damage if you move, as Bilić Vision Clinic uses state-of-the-art lasers with a built-in safety system that stops the laser if any movement is detected.
During the procedure, you may hear sounds and see various light effects, but you won’t feel any pain.
After the surgery, you’ll rest for about 30 minutes with your eyes closed so the corneal flap can settle back into place.
You’ll then receive antibiotic drops, artificial tears, and detailed verbal and written instructions on how to care for your eyes, use your medication, and attend follow-up check-ups.
Most patients already see quite well immediately after the procedure, although slight blurriness is normal on the first day. The entire visit to the clinic lasts around 90 minutes, ensuring expert and safe care every step of the way.
